Three months to start, because a specialist is not productive on your account before then and billing you for a shorter run would be selling you something that cannot work. After the initial term it is monthly rolling with 30 days' notice.
No — media spend is paid directly by you to the platform, so you own the accounts and the data. Management of that spend is included up to the threshold on your plan.
